Whether the individual you’re referring is coming from homelessness, hospitalization, an unsafe environment, or simply needs community and support, our mission is to provide a warm home, quality care, and a faith-filled atmosphere that empowers each resident to thrive.

Please complete the form below with as much accurate information as possible to help us determine if the applicant is a good fit for Simon Collaborative Living. This form takes 3–5 minutes and requires basic details about the applicant’s living situation, financial ability, sobriety, mental health needs, and ADL (daily living) support.

Once submitted, our team will review the referral and follow up within 24–48 hours with next steps.
